Dr. Kim Kuebler is a national/international thought leader in chronic conditions, pain, and palliative care. She received her Doctor of Nursing Practice degree from Vanderbilt University. Master of Nursing, Adult Nurse Practitioner and Clinical Specialist in Oncology from Emory University. Inducted Fellow into the American Academy of Nursing. She is a Mayday Pain Fellow from the Department of Neurology at Memorial Sloan Kettering Cancer Center in New York. Kuebler has held multiple state and federal appointments that include the Michigan Governors Commission on end-of-life care, four appointments to the Centers for Medicare and Medicaid Evidence Advisory Committee, grant reviewer for Patient Centered Outcomes Research Institute on comparative effectiveness research on chronic disease, pain and palliative care. Stakeholder for the Patient-Centered Care Planning for persons with multiple chronic conditions initiative through the Agency for Healthcare Policy and Research. Kuebler is an award-winning author of 8 textbooks on chronic disease, symptoms, and palliative care. Author of multiple peer-review publications and board member of the American Nursing Journal. She is the Editor-in-Chief for the Multiple Chronic Conditions Resource Center and is clinically active as an advanced practice provider over four decades. She advocates for best clinical practice through evidence, education, health policy and patient preferences.

Laly Havern, PharmD, MS, BCACP is a director on the Clinical Pharmacy Strategy team at Walgreens, where she leads the strategy for clinical products and services in Oncology, Reproductive Health, and Transplant. Her previous roles include 11 years in the field as a community pharmacist and 4 years in the clinical office conducting evidence-based clinical reviews and establishing clinical programs to address the needs of patients, providers, and partners. Her passion lies in surrounding patients with support through their health journey and improving clinical outcomes. She holds a doctorate in pharmacy from Duquesne University, is board-certified in ambulatory care, and a Master of Science in Pharmacogenomics from the University of Florida.
